These materials include the survey forms used as working copies by field workers throughout Kentucky, and the transcribed data cards for thirty-nine counties. Both records include personal information, such as, the veteran's name, birth and death dates, war in which he served, rank, branch of the service, and enlistment and discharge dates. In addition the county, cemetery name, location of the grave, and source of the information (headstone and/or relative) is recorded. The data cards may also include a rough sketch of the grave's location within the cemetery.
